Not the worst Sailor Moon cosplay ever (I hope ^^), but Sailor Venus in her disguise as Sailor Moon from episode 102 of the original anime.
After Usagi has been found out by Kaolinite, Minako uses the Disguise Pen to look like Sailor Moon. The disguise isn't perfect though, parts of her original (orange) uniform still show.
But with the help of the other Sailors and the cats, she convinces Kaolinite that she got the wrong girl, thus saving Usagi. - Date: 07/26/2014
