The Insane title is the most difficult to get. I would post a link to a website that guides you through this most insane feat of strength, but with only a month before the next patch hits, people just starting to pursue the Insane title will not make it on time. Unfortunately, Blizzard is removing two reputations you need for the Insane title. Sad, sad.

I got the Exalted a few weeks ago, and haven't really played my main since then. It's like the server went dead after the release date of Cata was announced. I'm going for the 100 companions achievement when it hits the stores, since I'm a sucker for lil' pets sweatdrop
