It depends on the game actually.
On Sims 3, I liked to play pre-made households. But im currently going a legacy with a custom household so I wont be playing pre-mades for a while.
On Sims 2 I don't usually play with the pre-made households at all. I like to start from an empty town, so I delete (not kill) all the townies. I played the Goth family for a little bit but thats it.
On Sims Life Stories I just leave the townies alone. But I want to make a town from scratch again on that game so I think I will delete the townies, except my friend's households. She plays with me, and I don't want to delete her stuff.
And on Sims 1 I used to love playing the Newbie household. Now, not so much. I made them have a cute baby named Futatsu.
On The Sims Medieval, I do EVERYTHING with the townies. I've fought them, dueled with them, got them arrested, flirted with them, made out with them, got pregnant with their baby...some of the women on that game are so pretty I take like 34083408 pictures of them but the boys look weird so I just have fun dueling with them. I used to make lesbians on the sims medieval all the time but I wanted them to have babies really bad, so...yeah.
