When you dream about the Sims, and you being one.

When you design a lot to look like your actual house (complete with the actual furniture) in the Sims

When you begin to think of your relationships with other people in terms of long term and short term bars... and imagine the hearts/happy faces by them...

You dance to the tunes on the little sim stereo. AND you know the words.

I do that! I have dreams about the Sims when I'm a Sim, my friends are Sims but my Sims are there as well, and everyone has green plumbobs floating above their head

~when you start making over all of the NPCs (my social workers: hooker, bodybuilder drag queen, social bunny)

.....when I actually take a lot of time using the grid paper and mapping out the architecture of my Sim's house, plan walking spaces and cramping spaces, the price of furniture and lifestyle, balancing having an exotic display piece with useful things, determine the color of furnishing for each and every room, designing each and every space in the house, plotting extensions from the small house and even reading the newspapers for discounts and purposely having my Sims attending sports events with the family for a fun time out.
