First off, that's a human body, not a "tentacle". If you've ever seen the Silent Hill movie, that's the body of one of the religious followers of Dahlia. Second, Pyramid Head actually has shoes, and I'm most certain his belt doesn't contain a cute little skull on it.

- If I may ask, what's with the third arm coming out of your right shoulder? Last I checked Pyramid Head couldn't summon forth extra arms, but it's still an awesome avatar.
